The office is open from 8:00am to 5:00pm Monday
through Friday.
To schedule an appointment, please call (512)
869-8500.
Please bring the following items during
your first visit:
- -Completed forms
- -Insurance card
- -Government issued photo ID (ex. Driver's License)
- -All prescription medication bottles
On subsequent visits please have the following
with you:
- -Insurance card.
- -Change of name, address, phone (home or cell),
employment and/or insurance.
Walk-in Clients:
- -Every effort will be made to accomodate the patient
the same day.
- -Subsequent appointment(s) may need to be made based
on the evaluation.
